Revised Payment Authorization Form

Attention campus users:

The University of Iowa Foundation is replacing its payment authorization form with a new Web-based form. To access this form, you will need to have a user account for the UIF Finance Department site.

• If you already have an account, you may access the form here.
• If you do not have an account, you will need to print and fill out this PDF document, then have it signed by the account representative with disbursement authority in your department.
