Instructions for Giving Appreciated Securities
with Stock Certificates Held by a Bank or Broker

It is important that you notify the UI Foundation in advance of making a gift of securities. Without such notice, gifts of stocks may be transferred to the Foundation with the identity of the donor unknown, making proper credit and timely acknowledgement extremely difficult.

Please use the form below. After you press SUBMIT, you will be able to print or save Depository Trust Company (DTC) instructions that provide delivery instructions for your banker/broker.

Note: If you don't want to use this online form, either you or your broker can contact UI Foundation's Account Services Coordinator at 319-335-3305 or stock-gifts@uifoundation.org to report what is being given and to receive instructions on how to complete the transfer of shares.
